Multivessel Coronary Artery Disease and Left main Coronary Artery Disease- CABG or PCI?

  • In this analysis of individual patient data from 11 randomized trials involving 11, 518 patients selected by heart teams to assign to percutaneous coronary intervention (PCI) (n=5753) or to coronary artery bypass grafting (CABG) (n=5765), 5 year all-cause mortality was significantly more with PCI (11·5%) than with CABG (8·9%) in patients with multivessel disease. (HR 1·28, 95% CI 1·09–1·49; p=0·0019).
  • CABG had a mortality benefit over PCI in patients with multivessel disease, particularly those with diabetes and higher coronary complexity, however, no benefit for CABG over PCI was seen in patients with left main disease.
